健脾补肾化痰方对 PCOS 伴 IR 患者 HOMA–IR 和 UA 的影响

〔摘 要〕 目的：研究健脾补肾化痰方对多囊卵巢综合征（PCOS）伴胰岛素抵抗（IR）患者的稳态模型胰岛素抵抗指数 （HOMA–IR）和血尿酸（UA）的影响。 方法：选取 2016 年 1 月至 2019 年 12 月龙岗区第二人民医院中医科门诊的 64 例 PCOS 伴 IR 患者，随机分为观察组和对照组，各 32 例。观察组给予中药健脾补肾化痰方治疗，对照组给予炔雌醇环丙 孕酮片联合二甲双胍治疗，治疗 3 个月。观察比较两组患者治疗前后月经情况及激素水平的变化，两组治疗总有效率。 结果：观察组患者治疗总有效率为 87.09 % 与对照组的 86.20 % 比较，差异无统计学意义（P ＞ 0.05）；治疗后两组患者的 HOMA–IR、身体质量指数（BMI）、黄体生成激素 / 卵泡刺激素（LH/FSH）、睾酮（T）水平均下降，与治疗前比较，差 异具有统计学意义（P ＜ 0.05）；且观察组患者 T 下降幅度较对照组少，观察组患者 UA 水平下降幅度较对照组更为显著， 差异具有统计学意义（P ＜ 0.05）；两组患者治疗后 HOMA–IR 水平比较，差异具有统计学意义（P ＜ 0.05）。 结论：健 脾补肾化痰方能减少 PCOS 伴 IR 患者 IR 和降低 UA 水平。

Abstract:

〔Abstract〕 Objective Observation of effi cacy of traditional Chinese medicine Jianpi Bushen Huatan Fang on the HOMA–IR and UA in patients with polycystic ovary syndrome comorbid insulin resistance. Methods From January 2016 to December 2019, 64 patients with PCOS and IR in the Chinese medicine department of the second people's hospital of Longgang district were selected and randomly divided into observation group and control group, each with 32 cases. Treatment group was given the traditional Chinese medicine. Control group was given Diane-35 combined therapy with metformin hydrochloride, 3 months is 1 treatment course. Then observe the changes of menstruation and Hormone Variety before and after treatment. Results According to Guiding Principle of Clinical Research of New Traditional Chinese Medicine, the treatment of menstruation was effective for 25 cases (86.20%) in control group and 27 cases (87.09%) in the treatment group. The menstrual symptoms of treatment group and control group were significantly improved .HOMA–IR, BMI, LH/FSH, T hormone decreased obviously (P < 0.05), T hormone of the control group decline signifi cantly, UA of the treatment group decreased signifi cantly, There is signifi cant difference of HOMA–IR between two groups after treatment (P < 0.05). Conclusion The results showed that Jianpi Bushen Huatan Fang can reduce insulin resistance in patients with PCOS Comorbid IR and lower uric acid level.
