大黄附子汤对脓毒症患者胃肠功能的促进作用

〔摘 要〕 目的：研究在脓毒症患者的治疗中采用大黄附子汤对其胃肠功能的促进作用。方法：选取河南省人民医 院 2017 年 3 月至 2019 年 3 月期间收治的 84 例脓毒症患者，依据随机数字表法将其分为观察组和对照组，各 42 例。 两组均进行营养支持、脏器支持及抗炎治疗，对照组采取西药治疗，观察组采用中药大黄附子汤治疗，比较两组治疗 前后的中医证候总积分、急性生理与慢性健康评分（APACHE Ⅱ）、胃肠功能障碍评分及治疗总有效率。结果：治 疗前两组患者中医证候总积分、APACHE Ⅱ评分、胃肠功能障碍评分比较，差异无统计学意义（P ＞ 0.05）；治疗后， 两组患者的中医证候总积分、APACHE Ⅱ评分、胃肠功能障碍评分均低于对照组，差异具有统计学意义（P ＜ 0.05）。 观察组患者治疗总有效率为 95.24 %，高于对照组的 76.19 %，差异具有统计学意义（P ＜ 0.05）。结论：对脓毒症患 者胃肠功能的治疗中，采用大黄附子汤治疗，可有效改善患者的临床症状，促进胃肠功能恢复。

Abstract:

〔Abstract〕Objective To study the promoting effect of dahuangfuzi decoction on gastrointestinal function in the treatment of sepsis patients. Methods A total of 84 patients with sepsis admitted to Henan Provincial People's Hospital from March 2017 to March 2019 were selected and divided into observation group and control group according to random number table method, with 42 cases in each group. The control group was treated with western medicine, and the observation group was treated with dahuangfuzi decoction. The total score of traditional Chinese medicine (TCM) syndrome, acute physiology and chronic health score (APACHE Ⅱ ), gastrointestinal dysfunction score before and after treatment and total effective rate were compared between the two groups. Results Before treatment, there was no significant difference in the total score of TCM syndrome, APACHE Ⅱ score and gastrointestinal dysfunction score between the two groups (P > 0.05). After treatment, the total score of TCM syndrome, APACHE Ⅱ score and gastrointestinal dysfunction score in the two groups were lower than those in the control group, the differences were statistically significant (P < 0.05). The total effective rate in the observation group was 95.24%, higher than 76.19% in the control group, the difference was statistically significant (P < 0.05). Conclusion In the treatment of gastrointestinal function of patients with sepsis, dahuangfuzi decoction can effectively improve the clinical symptoms of patients, promote the recovery of gastrointestinal function.
