加味清眩降压汤结合西药治疗阴虚 阳亢型高血压的疗效观察

Observation on the Curative Effect of Jiawei Qingxuan Jiangya Decoction Combined with Western Medicine in the Treatment of Hypertension with Yin Deficiency Yang Hyperactivity Type

〔摘 要〕 目的：探讨阴虚阳亢型高血压（HTN）应用加味清眩降压汤结合西药治疗的临床疗效。方法：选择河南 省省立医院2019年6月至2020年6月期间收治的98例阴虚阳亢型HTN患者，采用随机数字表法分为对照组与观察组， 各 49 例。对照组给予常规西药治疗，观察组在对照组的基础上给予加味清眩降压汤治疗。比较两组临床疗效、血清 学指标〔肿瘤坏死因子 –α（TNF–α）、白细胞介素 –6（IL–6）和同型半胱氨基酸（Hcy）〕、血压水平〔舒张压（DBP） 和收缩压（SBP）〕、血脂水平〔低密度脂蛋白（LDL–C）、总胆固醇（TC）、高密度脂蛋白（HDL–C）和三酰甘 油（TG）〕及安全性。结果：观察组患者治疗总有效率为 97.96 %，高于对照组的 79.59 %，差异具有统计学意义 （P ＜ 0.05）。治疗后两组患者的 IL–6、TNF–α、Hcy 均有不同程度下降，且观察组的 IL–6、TNF–α、Hcy 均低于对照组， 差异具有统计学意义（P＜0.05）。治疗后两组患者的DBP、SBP均有不同程度下降，且观察组的DBP、SBP低于对照组， 差异具有统计学意义（P ＜ 0.05）。治疗后两组患者的 LDL–C、TG、TC 均有不同程度下降，HDL–C 有不同程度升高， 且观察组的 LDL–C、TG、TC 均低于对照组，HDL–C 高于对照组，差异具有统计学意义（P ＜ 0.05）。两组患者治 疗过程中未见不良反应。结论：阴虚阳亢型 HTN 患者应用加味清眩降压汤结合西药治疗利于降低血压和血脂，减轻 炎症反应，且安全性高。

Abstract:

〔Abstract〕 Objective To investigate the clinical effect of application Jiawei Qingxuan Jiangya decoction combined with western medicine in the treatment of Yin deficiency Yang hyperactivity hypertension (HIN). Methods A total of 98 HBP patients with Yin deficiency Yang hyperactivity treated in Henan Provincial Hospital from June 2019 to June 2020 were selected and divided into control group and observation group by random number table method, with 49 cases in each group. The control group was treated with conventional western medicine treatment, and the observation group was treated with Jiawei Qingxuan Jiangya decoction on the basis of the control group. The clinical efficacy, serological indexes [tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and homocysteine (Hcy)], blood pressure levels [diastolic blood pressure (DBP) and systolic pressure (SBP), blood lipid levels [low density lipoprotein (LDL-C), total cholesterol (TC), high density lipoprotein (HDL-C) triacylglycerol (TG)] and safety were compared between the two groups. Results The total effective rate of the observation group was 97.96%, higher than 79.59% of the control group, the difference was statistically significant (P < 0.05). After treatment, the levels of IL-6, TNF-α and Hcy in the two groups were decreased to varying degrees, and the levels of IL-6, TNF- α and Hcy in the observation group were lower than those in the control group, the differences were statistically significant (P < 0.05). After treatment, the DBP and SBP in the two groups decreased to varying degrees, and the DBP and SBP in the observation group were lower than those in the control group, the differences were statistically significant (P < 0.05). After treatment, LDL-C, TG and TC in the two groups decreased to varying degrees, while HDL-C increased to varying degrees. LDL-C, TG and TC in the observation group were lower than those of the control group, and HDL-C in the observation group was higher than that of the control group, the differences were statistically significant (P < 0.05). There were no adverse reactions in the two groups during treatment. Conclusion The application of Jiawei Qingxuan Jiangya decoction combined with western medicine in HTN patients with Yin deficiency and Yang hyperactivity is beneficial to reduce blood pressure and blood lipids, reduce inflammatory reaction, and has high safety.
