解毒固肾汤联合血液透析对终末期肾病 患者微炎症状态、肾功能的影响

Effects of Jiedu Gushen Decoction Combined with Hemodialysis on Micro-inflammatory State and Renal Function in Patients with End-stage Renal Disease

〔摘 要〕 目的：探讨解毒固肾汤联合血液透析对终末期肾病（ESRD）患者微炎症状态、肾功能的影响。方法： 选取洛阳市第三人民医院 2020 年 4 月至 2021 年 6 月期间收治的 84 例 ESRD 患者，按照随机抽签法分为观察组和 对照组，各 42 例。观察组患者采用解毒固肾汤联合血液透析进行治疗，对照组患者采用血液透析法进行治疗。治疗 3 个月后，比较两组患者治疗前后中医症状（倦怠乏力、腰膝酸软、恶心呕吐）的疗效评价，比较两组患者治疗前后 肾功能〔血清尿素氮（BUN）、血肌酐（SCr）、尿酸（UA）〕水平变化，微炎症状态〔白细胞介素 –6（IL–6）、 C 反应蛋白（CRP）〕水平变化。结果：观察组患者治疗总有效率为 88.09 %，高于对照组的 66.67 %，差异具有统计 学意义（P ＜ 0.05）；治疗后两组患者的 BUN、SCr、UA、IL–6、CRP 均有不同程度下降，且治疗后观察组患者的 BUN、SCr、UA、IL–6、CRP 明显低于对照组，差异具有统计学意义（P ＜ 0.05）。结论：解毒固肾汤联合血液透析 可有效改善 ESRD 患者微炎症状态及肾功能，提高其免疫功能，更有利于患者预后恢复。

Abstract:

〔Abstract〕 Objective To explore the effects of Jiedu Gushen decoction combined with hemodialysis on micro-inflammatory state and renal function in patients with end-stage renal disease. Methods 84 patients were selected and divided into an observation group and a control group with 42 cases in each group by the random drawing method. The experimental group was given Jiedu Gushen decoction combined with hemodialysis, while the control group was treated with hemodialysis. After 3 months of treatment, the efficacy evaluation of traditional Chinese medicine (TCM) symptoms (languid, lumbar debility, nausea and vomiting) before and after treatment were compared between the two groups of patients. The changes in renal function [blood urea nitrogen (BUN), serum creatinine(SCr), uric acid (UA)], [interleukin-6 (IL-6), C-reactive protein (CRP)] of the two groups of patients were compared before and after treatment. Results The total effective rate of the observation group was 88.09 %, higher than 66.67 % of the control group, the difference was statistically significant (P < 0.05). After treatment, the levels of BUN, SCr, UA, IL-6 and CRP in the two groups decreased to varying degrees, and the levels of BUN, SCr, UA, IL-6 and CRP in the observation group were significantly lower than those in the control group, the differences were statistically significant (P < 0.05). Conclusion Jiedu Gushen decoction combined with hemodialysis can effectively improve the microinflammatory state and renal function of ESRD patients, improve their immune function, and is more conducive to the prognosis and recovery of patients.
