Application of Socket Shield Technique in Immediate Implantation of Single Missing Teeth in Anterior Molars Region

摘要:

〔摘 要〕 目的：探究盾构技术（SST）在即刻种植术治疗上前磨牙区单牙缺失患者中的应用价值。 方法：选取厦 门市思明区厦港街道社区卫生服务中心 2018 年 1 月至 2020 年 5 月收治的 25 例上前磨牙区单牙种植患者，随机数字 表法将患者分为两组，观察组 13 例实施 SST 联合即刻种植术，对照组 12 例单独实施即刻种植术。随访 1 年两组均 无脱落病例，比较两组手术前后软组织美观度、疼痛程度、生活质量、唇侧骨板厚度变化情况及种植体稳定性。 结果： 术后 1 年，观察组视觉模拟评分法（VAS）评分低于对照组，红色美学指数（PES）评分高于对照组，差异具有统计 学意义（P ＜ 0.05）；术后即刻至 1 年，观察组患者的 1 mm–IS、3 mm–IS、5 mm–IS 骨板厚度变化量均小于对照组， 差异具有统计学意义（P ＜ 0.05）；术后 1 年，观察组患者种植体稳定系数（ISQ）评分高于对照组，差异具有统计 学意义（P ＜ 0.05）；两组患者术后 1 年口腔健康生活质量量表（OHIP–20）评分较术前降低，且观察组低于对照组， 差异具有统计学意义（P ＜ 0.05）。 结论：SST 联合即刻种植术治疗上前磨牙区单牙种植患者能减轻唇侧骨板吸收， 提高种植体稳定性，有助于缓解疼痛，增强修复美观度，提高患者生活质量。

Abstract:

〔Abstract〕 Objective To explore the application value of socket shield technique (SST) in immediate implantation for the treatment of single tooth loss in the anterior molars. Methods A total of 25 patients with single dental implantation in the anterior molar area who were admitted to Xiamen Siming District Xiagang Street Community Health Service Center from January 2018 to May 2020 were selected and divided into two groups by random number table method. The observation group with 13 cases received SST combined with immediate implantation, and the control group with 12 cases received immediate implantation alone. The soft tissue aesthetics, pain, quality of life, labial bone plate thickness changes and implant stability were compared between the two groups before and after operation. Results One year after operation, the visual analogue scale (VAS) score of the observation group was lower than that of the control group, and the pink esthetic score (PES) was higher than that of the control group (P < 0.05). The changes of bone plate thickness of 1 mm-IS, 3 mm-IS and 5 mm-IS in the observation group were smaller than those in the control group immediately after operation (P < 0.05). One year after operation, the implant stability quotient (ISQ) score of the observation group was higher than that of the control group (P < 0.05). The oral health impact profile (OHIP-20) score of the two groups at 1 year after operation was lower than that before operation, and the score of the observation group was lower than that of the control group (P < 0.05). Conclusion SST combined with immediate implantation in the treatment of patients with single dental implant in the anterior molars can reduce the absorption of labial bone plate, improve the stability of the implant, help to relieve pain, enhance the aesthetics of restoration, and improve the quality of life of patients.
