腹腔镜联合肠镜在小儿宽大基底部 结肠息肉摘取术中的应用效果

〔摘 要〕 目的：观察腹腔镜联合肠镜在小儿宽大基底部结肠息肉摘取术中的应用效果。 方法：选取 2018 年 6 月 至 2022 年 6 月期间在河南省儿童医院接受手术治疗的 46 例宽大基底部结肠息肉患儿，将入选患儿按随机数字表法均 分为单纯组与联合组，每组 23 例。单纯组采用单纯腹腔镜下息肉摘取术治疗，联合组采用腹腔镜联合肠镜下息肉摘 取术治疗。比较两组患儿临床指标及术前、术后血清炎症因子与相关标志物水平的变化、症状改善情况。 结果：联合 组患儿手术用时、术中出血量、胃肠功能恢复时间及总住院时间均少于单纯组，差异具有统计学意义（P ＜ 0.05）； 联合组患儿手术后各项血清炎症因子与相关标志物水平均低于单纯组，差异具有统计学意义（P ＜ 0.05）；两组患儿 手术前后各项症状评分比较，差异无统计学意义（P ＞ 0.05）。结论：腹腔镜联合肠镜应用于小儿宽大基底部结肠息 肉摘取术中，能够更彻底地切除息肉，同时减轻患儿术后炎症反应程度。

Abstract:

〔Abstract〕 Objective To observe the application effect of laparoscopy combined with colonoscopy in the removal of large basal colon polyps in children. Methods A total of 46 children with large basal colic polyps who received surgical treatment in Henan Children's Hospital from June 2018 to June 2022 were selected and divided into a simple group and a combined group according to random number table method, with 23 cases in each group. The simple group was treated with simple laparoscopic polypectomy, and the combined group was treated with laparoscopic polypectomy combined with colonoscopy. The clinical indicators, the changes of serum inflammatory factors and related markers before and after surgery, and the improvement of symptoms were compared between the two groups. Results The operation time, intraoperative blood loss, gastrointestinal function recovery time and total hospital stay in the combination group were all less than those in the simple group, and the differences were statistically significant (P < 0.05). The levels of serum inflammatory factors and related markers in the combination group were lower than those in the simple group after surgery, and the differences were statistically significant (P < 0.05). There was no significant difference in symptom scores between the two groups before and after operation (P > 0.05). Conclusion The application of laparoscopy combined with colonoscopy in the removal of large basal colon polyps in children can remove polyps more thoroughly and reduce the degree of postoperative inflammation in children.
