苦百洗剂联合糠酸莫米松乳膏治疗 Ⅲ ~ Ⅳ 期 内痔合并肛门湿疹术后临床疗效

Clinical Effect of Kubai Lotion Combined with Mometasone Furoate Cream in the Treatment of Ⅲ ~ Ⅳ Hemorrhoids Complicated with Anal Eczema

〔摘 要〕 目的：探讨苦百洗剂联合糠酸莫米松乳膏治疗 Ⅲ ~ Ⅳ 期内痔合并肛门湿疹的临床效果。 方法：选取 上海中医药大学附属第七人民医院 2018 年 9 月至 2019 年 8 月期间肛肠科住院的 100 例 Ⅲ ~ Ⅳ 期内痔合并肛门湿 疹患者，按照随机数字表法分为对照组与观察组，各 50 例。观察组患者术后采用苦百洗剂熏洗和糠酸莫米松软膏外 涂；对照组患者术后采用痔疾洗液熏洗和糠酸莫米松软膏外涂。比较两组患者总有效率，术后第 1、3、7、14 天肛门 水肿和疼痛情况，术前和术后第 7 天肛门瘙痒程度及频率。 结果：两组患者治疗总有效率比较，差异无统计学意义 （P ＞ 0.05）。术后第 1 天及术后第 3 天两组患者水肿评分比较，差异无统计学意义（P ＞ 0.05）；术后第 7 天和 第 14 天观察组患者的水肿评分低于对照组，差异具有统计学意义（P ＜ 0.05）。且两组患者的的水肿程度均在术后 第 7 天达到高峰，随后又开始好转。术后第 1 天两组患者视觉模拟评分法（VAS）评分比较，差异无统计学意义 （P ＞ 0.05）；术后第 3 天、第 7 天和第 14 天观察组患者的 VAS 评分低于对照组，差异具有统计学意义（P ＜ 0.05）。 且两组患者的的 VAS 评分均在术后第 7 天达到高峰，随后又开始好转。治疗后两组患者的瘙痒频率及程度评分均有 不同程度下降，且治疗后观察组患者的瘙痒频率及程度评分均低于对照组，差异具有统计学意义（P ＜ 0.05）。 结论：苦百洗剂联合糠酸莫米松乳膏应用于 Ⅲ ~ Ⅳ 期内痔合并肛门湿疹术后能有效减轻患者疼痛及水肿症状，且在治疗肛 周瘙痒上也有较好的疗效。

Abstract:

〔Abstract〕 Objective To investigate the clinical effect of Kubai lotion combined with mometasone furoate cream in the treatment of Ⅲ ~ Ⅳ hemorrhoids complicated with anal eczema. Methods A total of 100 patients with stage Ⅲ ~ Ⅳ hemorrhoids complicated with anal eczema admitted to the 7th People's Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from September 2018 to August 2019 were selected and divided into a control group and an observation group according to random number table method, with 50 cases in each group. The observation group was fumigated with Kubai lotion and coated with mometasone furoate ointment after surgery, the control group was fumigated with hemorrhoidal disease washing solution and coated with mometasone furoate ointment. The total effective rate, anal edema and pain at day 1, 3, 7 and 14 after surgery, the degree and frequency of anal pruritus before surgery and on day 7 after surgery were compared between the two groups. Results There was no significant difference in the total effective rate between the two groups (P > 0.05). There was no significant difference in edema score between the two groups on day 1 and day 3 after surgery (P > 0.05). The edema score of the observation group was lower than that of the control group on the 7th and 14th day after surgery, and the difference was statistically significant (P < 0.05). In both groups, the degree of edema peaked on the 7th day after surgery, and then began to improve. There was no significant difference in visual analog scale (VAS) scores between the two groups on day 1 after surgery (P > 0.05). The VAS scores of the observation group were lower than those of the control group on day 3, 7 and 14 after surgery, and the differences were statistically significant (P < 0.05). The VAS scores of the two groups reached the peak on the 7th day after surgery and then began to improve again. After treatment, the itching frequency and degree scores of the two groups were decreased to varying degrees, and after treatment, the itching frequency and degree scores of the observation group were lower than those of the control group, the differences were statistically significant (P < 0.05). Conclusion Kuobei lotion combined with mometasone furoate cream can effectively reduce the pain and edema symptoms of patients after the operation of hemorrhoids in stage Ⅲ ~ Ⅳ complicated with anal eczema, and has a good effect on the treatment of perianal pruritus.
