经阴道彩色多普勒超声对子宫腺肌瘤的诊断价值

Diagnostic Value of Transvaginal Color Doppler Ultrasound in Uterine Adenomyoma

〔摘 要〕 目的：探讨经阴道彩色多普勒超声对子宫腺肌瘤的诊断价值。 方法：选取许昌中医院 2020 年 1 月至 2022 年 1 月期间收治的 80 例子宫腺肌瘤患者为研究对象，对上述患者实施经阴道彩色多普勒超声诊断，获得对应的 诊断结果，随后将该结果和术后病理分析结果进行比较，最终得出诊断效果。 结果：本研究 80 例子宫腺肌瘤患者中， 经阴道彩色多普勒超声检出 75 例，漏诊或误诊 5 例，诊断准确率为 93.75 %；经阴道彩色多普勒检查诊断为子宫腺 肌瘤的 75 例患者中，按照发病部位划分，有 10 例（13.33 %）子宫腺肌瘤出现在子宫前壁区域，65 例（86.67 %） 出现在子宫后壁部位；按照病情特点划分，有 63 例（84.00 %）为单发子宫腺肌瘤，12 例（16.00 %）为子宫腺肌瘤 合并子宫肌瘤。此外，75 例患者中有 3 例患者并发卵巢内膜异位症。75 例患者的子宫动脉血流阻力指数为 0.75 ± 0.05、 搏动指数为 1.37 ± 0.04、舒张期峰值速度为（5.83 ± 0.14）cm·s -1、收缩期峰值速度为（71.74 ± 0.25）cm·s -1 。 结论：经阴道彩色多普勒超声可通过超声特征、血流分布、多普勒频谱特点，准确诊断子宫腺肌瘤。

Abstract:

〔Abstract〕 Objective To explore the diagnostic value of transvaginal color Doppler ultrasound in uterine adenomyoma. Methods 80 cases of uterine adenomyoma treated in Xuchang Hospital of Traditional Chinese Medicine from January 2020 to January 2022 were selected as the research object, and the corresponding diagnosis results were obtained by transvaginal color Doppler ultrasound diagnosis. Then, the results were compared with the results of postoperative pathological analysis, and finally the diagnosis effect was obtained. Results Among 80 cases of uterine adenomyoma in this study, 75 cases were detected by transvaginal color Doppler ultrasound, and 5 cases were missed or misdiagnosed. The diagnostic accuracy rate was 93.75%. Among 75 cases diagnosed as uterine adenomyoma by transvaginal color Doppler, according to the location of the disease, 10 cases (13.33%) of uterine adenomyoma appeared in the anterior wall of uterus and 65 cases (86.67%) in the posterior wall of uterus. According to the characteristics of the disease, 63 cases (84.00%) were single adenomyoma, and 12 cases (16.00%) were adenomyoma combined with hysteromyoma. In addition, 3 of 75 patients were complicated with ovarian endometriosis. In 75 patients, the uterine artery blood flow resistance index was 0.75 ± 0.05, the pulsatility index was 1.37 ± 0.04, the diastolic peak velocity was (5.83 ± 0.14) cm·s -1, and the systolic peak velocity was (71.74 ± 0.25) cm·s -1. Conclusion Transvaginal color Doppler ultrasound can accurately diagnose uterine adenomyoma through ultrasonic characteristics, blood flow distribution and Doppler spectrum characteristics.
