自拟稳心汤治疗心房颤动患者的临床分析

Clinical Effect of Self-Designed Wenxin Decoction on Patients with Atrial Fibrillation

〔摘 要〕 目的：分析自拟稳心汤加减对心房颤动（AF）患者心电图与临床指标的影响。 方法：回顾性分析常德 市第一中医医院 2021 年 1 月至 2021 年 12 月期间收治的 AF 患者，常规治疗的 46 例患者纳入对照组，常规治疗基础 上应用自拟稳心汤加减治疗的 46 例患者纳入观察组。比较两组患者治疗前后 AF 发作频率、持续时间，中医主证、 次证、舌脉积分以及心电图特征〔P 波最大时限（Pmax）、P 波最小时限（Pmin）、P 波离散度（Pd）、P 波变异（Pv）、 R 波间期（R–R）〕差异度的变化情况。 结果：治疗后两组患者发作频率、持续时间均有不同程度减少，且治疗后观 察组患者发作频率、持续时间均少于对照组，差异具有统计学意义（P ＜ 0.05）。治疗后两组患者各项中医证候积分 均有不同程度减少，且治疗后观察组患者各项中医证候积分均低于对照组，差异具有统计学意义（P ＜ 0.05）。治疗 后两组患者 Pmax、Pd、Pv 均有不同程度降低，R–R 升高；且治疗后观察组患者 Pmax、Pd、Pv 均低于对照组，R–R 高于对照组，差异具有统计学意义（P ＜ 0.05）。两组患者均未见影响治疗的严重不良反应。 结论：自拟稳心汤加减 能够进一步减少 AF 的发作频率与持续时间，减轻患者各项中医证候，改善心电图指标，且用药安全可行。

Abstract:

〔Abstract〕 Objective To analyze the effects of modified Wenxin Decoction on electrocardiogram and clinical indexes in patients with atrial fibrillation (AF). Methods Retrospective analysis was performed on AF patients admitted to Changde First Hospital of Traditional Chinese Medicine from January 2021 to December 2021. 46 patients receiving conventional treatment were included in the control group, and 46 patients receiving modified treatment with self-designed Wenxin decoction on the basis of conventional treatment were included in the observation group. The frequency and duration of AF attacks, the main and secondary syndroms of Traditional Chinese Medicine (TCM), tongue and pulse integrals, and the variation of ECG characteristics [P wave maximum time limit (Pmax), P wave minimum time limit (Pmin), P wave dispersion (Pd), P wave variation (Pv), R–R interval (R-R)] were compared between the two groups before and after treatment. Results After treatment, the attack frequency and duration of the two groups were lower than those before treatment, and after treatment, the attack frequency and duration of the observation group were lower than those of the control group, the differences were statistically significant (P < 0.05). After treatment, the scores of various TCM syndroms of the two groups were lower than those before treatment, and after treatment, the scores of various TCM syndroms of the observation group were lower than those of the control group, the differences were statistically significant (P < 0.05). After treatment, Pmax, Pd, and Pv in both groups decreased to varying degrees, with R-R higher than that before treatment. Moreover, after treatment, Pmax, Pd, and Pv in the observation group were lower than those in the control group, with R-R higher than that in the control group, the differences were statistically significant (P < 0.05). There were no serious adverse reactions affecting the treatment in both groups. Conclusion Self-designed Wenxin decoction can further reduce the frequency and duration of atrial fibrillation, alleviate various TCM syndromes of patients, improve electrocardiogram indexes, and the drug is safe and feasible.
