聂勇主任医师治疗甲状腺良性结节 合并桥本甲状腺炎经验分析

Analysis of the Experience of Chief Physician Nie Yong in Treating Benign Thyroid Nodules Complicated with Hashimoto’s Thyroiditis

〔摘 要〕 目的：研究聂勇主任医师自拟消瘿方治疗甲状腺良性结节合并桥本甲状腺炎患者对其甲状腺抗体、甲状腺结 节大小数量的影响。 方法：选取驻马店市中医院 2022 年 5 月至 2023 年 5 月期间诊治的 92 例甲状腺良性结节合并桥本甲状 腺炎患者作为研究对象，随机分为对照组与观察组，各 46 例。其中对照组患者给予左甲状腺素钠片治疗，观察组患者在对 照组基础上给予自拟消瘿方治疗。治疗 4 个月，观察两组患者临床疗效、中医证候积分、甲状腺抗体、甲状腺结节大小数 量以及不良反应。 结果：观察组患者治疗总有效率高于对照组；观察组患者治疗后中医证候积分低于对照组；观察组患者 治疗后血清甲状腺过氧化物酶抗体（TPOAb）、甲状腺球蛋白抗体（TGAb）水平均低于对照组；观察组患者治疗后甲状腺 峡部厚度、两叶前后径、结节最长径、结节数目均低于对照组，以上差异均具有统计学意义（P ＜ 0.05）。两组患者不良 反应发生率比较，差异无统计学意义（P ＞ 0.05）。 结论：聂勇主任医师自拟消瘿方能够有效降低甲状腺良性结节合并桥 本甲状腺炎患者自身甲状腺抗体水平，抑制结节进展，缓解临床症状，安全性高。

Abstract:

〔Abstract〕 Objective To study the effect of Chief Physician Nie Yong’s self-formulated Xiaoying Prescription on thyroid antibodies, the size and number of thyroid nodules in patients with benign thyroid nodules complicated with Hashimoto’s thyroiditis. Methods A total of 92 patients with benign thyroid nodules complicated with Hashimoto’s thyroiditis diagnosed and treated in Zhumadian Traditional Chinese Medicine Hospital from May 2022 to May 2023 were selected as the research subjects and randomly divided into a control group and an observation group, with 46 cases in each group. The patients in the control group were treated with levothyroxine sodium tablets, while the patients in the observation group were treated with self-formulated Xiaoying Prescription on the basis of the control group. After 4 months of treatment, the clinical efficacy, traditional Chinese medicine (TCM) syndrome scores, thyroid antibodies, thyroid nodule size and number, and adverse reactions were observed in both groups. Results The total effective rate of treatment in the observation group was higher than that in the control group; the TCM syndrome score of the observation group was lower than that of the control group after treatment; the serum levels of thyroid peroxidase antibody (TPOAb) and thyroglobulin antibody (TGAb) in the observation group were lower than those in the control group after treatment; the thyroid isthmus thickness, anteroposterior diameter of the two lobes, the longest diameter of the nodules, and the number of nodules in the observation group were all lower than those in the control group after treatment, with statistically significant differences (P < 0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion Chief Physician Nie Yong’s self-formulated Xiaoying Prescription can effectively reduce the level of thyroid autoantibodies in patients with benign thyroid nodules complicated with Hashimoto’s thyroiditis, inhibit the progression of nodules, alleviate clinical symptoms, and has high safety.
